Our volunteers are very special to us. Serving the MUAA community and our students via volunteer work requires patience, time, and effort.

Download the appropriate form below. Follow the instructions and submit your completed forms with required documents to the school office.
Basic Volunteer

For Off-Campus Trips & Frequent Campus Volunteers
Volunteers who accompany students while on off-campus trips and/or are with students regularly on campus for no more than 32 hours in a school year. (Renew every two years)

Extended Volunteer

For Frequent Volunteers (32+ hours/yr) and Overnight Trips
Volunteers who have frequent or prolonged contact with students (32 hours or more for the school year). Examples include all chaperones on overnight school trips, coaches, and individuals volunteering on a weekly basis. (Requires LIVE SCAN—Form included)
